** COMING UP FOR 2010-2011 SCHOOL YEAR **

The State of Ohio now requires All students going into the 7th grade in the 2010-2011 school year to have a TD or Tdap prior to the first day of school.

For each new school year...

remember to send in the proper forms if any medications are needed.

We have added an epipen form that you may download!

We have compiled a list of questions below that you may have.

Does the clinic need my volunteer hours?

YES!

Volunteers-RN's, LPN's, paramedics, EMT needed at various times throughout the school year, also at screening times in the fall. Please call your clinic to schedule with us.

Your donations are also appreciated as we often need to restock our shelves of frequently used items such as:

 

7-UP or Sprite-small cans preferred

Soda crackers

Peppermints or Spearmints

Ziplock plastic bags of all sizes
